Gastro-Oesophageal Reflux (GORD)

Reflux is very common in babies and young children. Understanding whether it is simple reflux or GORD is key to finding the right management plan for your child.

What is GORD / Reflux?

Reflux is when a baby brings up milk, or is sick, during or shortly after feeding.

It is very common and usually gets better on its own.

Silent Reflux

Sometimes babies may have signs of reflux but will not bring up milk or be sick. This is known as silent reflux.

Symptoms of GORD

  • Bringing up milk or being sick during or shortly after feeding
  • Coughing or hiccupping when feeding
  • Being unsettled during feeding
  • Swallowing or gulping after burping or feeding
  • Crying and not settling
  • Not gaining weight as they're not keeping enough food down

What Happens in GORD?

In GORD the acid from the stomach leaks up into the oesophagus.

Normally, a ring of muscle at the bottom of the oesophagus — the lower oesophageal sphincter — acts as a valve, keeping stomach contents from flowing back up. In babies and children with GORD, this valve is immature or weakened, allowing stomach acid and contents to reflux upwards.

  • Lower oesophageal sphincter (valve) fails to close properly
  • Stomach contents flow backwards into the oesophagus
  • Acid causes irritation and discomfort

Management of GORD

Lifestyle & Feeding Measures

  • Smaller, more frequent feeds
  • Keeping baby upright after feeding
  • Thickening feeds (formula-fed babies)
  • Reviewing feeding technique
  • Dietary changes for breastfeeding mothers

Medications

  • Antacids
  • Alginates (e.g. Gaviscon Infant)
  • Proton pump inhibitors (PPIs)
  • H2 receptor antagonists

Investigations (if required)

  • pH / impedance study
  • Upper GI endoscopy
  • Barium swallow
